does anyone have the reanactment with the voice and the cartoon model of the incident. It played here awhile ago and I forgot to get it.

Privlaged information, sorry. The video got out a few months back and some people are in the process of getting their balls stomped by the AF. I think it's pretty ridiculous that the military doesn't share accident investigation information with the general public, especially ones that involved so many CRM issues as this one did.
 
The info wll be shared officially, but not for a while. The Air Force wants to conduct its own investigations and doesn't want to release any information to the public until they are complete. Nor should they. Gotta keep everyone impartial.
 
The AF conducts two investigations of major aircraft mishaps. The first investigation, the Safety Investigation Board, is usually conducted first. To encourage forthright and honest testimony, much of the investigation is conducted with promises of confidentiality to all involved. The second investigation, the Accident Investigation Board, is typically conducted second and offers no such promises. The AIB involves lawyers and appoints blame--and the results are public. The two boards don't always reach the same conclusions.

If every privileged animation/SIB finding made it into the public venue, mishap crews would cease cooperating with the SIB and all AF aircrews would no longer benefit from the critical lessons learned. So, help us out and stop sharing privileged info.

If the mil released more than a simple press release after determining probable cause, I'd agree. I don't know if what I read was eyes only material, but it was thought provoking from a professional standpoint. But for the grace of God, there go I. I want to learn about the mistakes made by others, not for some cheap thrill or out of morbid curiosity but rather so I can avoid making them in the future myself. Isn't that a tenant of any safety program? Yeah, I understand your point and its not without validity. The privacy aspect must be balanced with the benefits that come from an educated pilot group. I am not prepared for some watered down change to SOP's months if not years later to be the only insight in to a preventable accident.

Its seems they do? This months Flying reviews it in some detail. They take a pretty good swipe at the AC.
