I appreciate the rides in the past and you're always welcome on us. Last week I was stuck in LGA trying to get to YUL beacause three crews showed up without the Flight Deck Coupon (T410), which is required for off line pilots to travel.

Is it common to be short of these forms?

Is there a way to place a call in advance to ensure that the flight crew carry these forms?
