Anyone ever request these two reports? Any surprises? How long did it take to hear a response?

Yes, both.

Any surprises?
Yes, there was something on my NDR from when I was 16 (14 years ago). Almost fell out of my chair. It doesn't specify what the offense was, just refers you to which state reported it.

How long did it take to hear a response?
Airmen file took almost two months after I requested it.
NDR took about three weeks.

Mail request to: Airman Certification Branch, AFS - 760, PO Box 25082, Oklahoma City, OK 73125
Include Full name, Date of Birth, airman or social security number, current mailing address, signature, The words"I am requesting a copy of my complete FAA airman file", and a check for $10 made payable to U.S Treasury
