AF Form 24 PDF autofill????

There used to be a website, http://www.jagusaf.hq.af.mil/Brochures/AFFRM24.pdf, that had an autofill PDF version of AF Form 24. Does anyone know of a new link or another site? I don't have form flow and I'd prefer to stick with the PDF format.

Dont use that form. It is expired. Notice on the bottom you see 1998....... there is a new one that says 2002 or 2003....... then next to it, it says prior verion is omitted or expired. I forget. All i know is that is the old one. New one is the same but top right is a little different. Just my observation. Later.
